Requirements:

To obtain a copy of birth/death certificate, a search form must be completed and returned to the Kenya Embassy D.C.

Complete the Birth/Death Certificate Search Form and submit the completed form with the following:

1.  
   1. US $50 (or equivalent in Indonesian Rupiah) Money order or Cashiers Check Payable to Embassy of The Republic of Kenya.
   2. Copy of the original Birth/Death Certificate.
   3. A formal letter of application.
   4. A prepaid, self-addressed (addressed from the Embassy to your own address) DHL return envelope must be submitted with each application. Please ensure it has adequate postage.

When completed, this application should be sent to –

Note: Any incomplete application will be returned.
